Thats weird…the nurse led me to believe it was for ALL cancer patients across the province regardless of which cancer centre you were a patient of. There is no indication on their site that's it's PMH/UHN patients only.

CareChart (CareChart@home), a subsidiary of Bayshore HealthCare Ltd., is an after-hours virtual support program for patients undergoing cancer treatment.
Sponsored by Ontario Health (Cancer Care Ontario), this program provides symptom management support for oncology patients receiving treatment when their local treatment centre is closed. Between their cancer treatment centre and CareChart, patients have access to 24-hour care, 365 days a year to help them manage their symptoms while remaining at home.

This service is available 24/7 x 365 days/year.
I believe they have an app that can be used to connect with them as well.
You will be connected with an Oncology Nurse who can help you with your issue. A report will be issued and sent to your oncologist.
So, if you find yourself needing help when your care centre is closed, you can give them a call. On the first call, you will have to answer a bunch of questions in order to set up your account. Name, birthdate, health #, your hospital identifier number (if you have it), your oncologist's name…I think that's all.
I was trying to reach the trial oncologist on call and was put through to them by mistake. I have been at this for over 6 years and did not know this free service (funded by Ontario Health) was available. The nurse was very surprised that I wasn't aware of them. She was under the impression that their contact info was provided to patients.
I think most cancer care centres have their own after-hours call centre set up…so they may not pass this one along. In any case, I thought it may be a useful resource and wanted to share with you
